How to Apply for Services

For more information about any of the services and programs available at CCRI, please contact Shannon Bock, Director of Programs, or your county service coordinator. CCRI’s services are funded by your local county social service agency; therefore, a county service coordinator must authorize them. The county service coordinator will determine if you qualify for services. If you are not currently involved with a service coordinator, CCRI can assist you in establishing a contact.

Upon authorization of services, CCRI will conduct an initial assessment. With the help of the family, an individualized plan is written. This unique plan is developed to meet the needs of each person. We pride ourselves on providing individualized services, as no two people are alike, no two service packages are alike.
